Chronic Disease Management

Karen Barry, RN

Case Coordinator – Chronic Disease Management

Professionally, I am a Registered Nurse and Certified Diabetes Educator. I have worked at Red Deer PCN for 13 years as such and prior to that my work experience was in HD, ER, ICU and OR for a few more years than that most of which was in Red Deer and Lacombe.

I have enjoyed my chosen career in every area and continue to do so. I welcome education and learning from everyone I connect with.

Personally, I am a proud Mother of 2 married adult children and even prouder Grammy to 3 kidlets.

I am a new member of this most beautiful area of Mountain View County, thanks to my partner Brian. with whom I am sharing a wide variety of fun and adventure.

I love to travel, hike, ski, skate, toboggan, swim, take pictures – we generally work and play outside and can’t wait for motorcycle / gardening weather!!!

Thank you for the opportunity to work here with you all.

Donna Hopton, RN

Case Coordinator – Chronic Disease Management

I am a Registered Nurse, and received my training at Humber College in Ontario. After graduation I accepted a position at White County Hospital in Sparta Tennessee where I worked acute care and was then offered a Nursing Supervisory position. It was a tremendous opportunity and memorable adventure that lasted one year. From there I moved back to my home town of Strathmore, Alberta, however with no nursing jobs available there I found employment as a Personal Care Aid with the new Long Term Care facility opening in Sundre, AB. Moving to Sundre eventually led to employment as an RN. 4 years later a position became available at Greenwood Family Physicians where I worked until September of 2014. At this time the Peaks to Prairies Primary Care Network was hiring for a position in Sundre and I was fortunate to be offered that role. It has been a gift to remain living and working in Sundre all these years surrounded by family, good friends and a beautiful country home.

Amy enjoys everything outdoors, hiking and exploring this beautiful area with her furbabies and family.

Tammy Blackwell – Foot Care Nurse

My husband and I relocated to the Sundre area in July 2020 from Ontario.   The experience I bring with me is 10 years as a Visiting Home Care Nurse and 6 years as a Foot Care Nurse.  In Ontario, I operated my own mobile Foot Care business as well as a clinic through Shoppers Drug Mart at two store locations.  Prior to my Nursing career, I was the owner/operator of an Electrolysis business for 25 years.  I also have a background in Aviation Management, and worked as a Supervisor at Blue Mountain Resorts, in Ontario.  One of my favorite hobbies in Ontario was teaching Western horseback riding.  With so much to see and do in Alberta, my husband and I now enjoy camping in the Mountains and going for long drives on our Harleys.  I’m very excited to be part of the Peaks to Prairies team and looking forward to setting up the Foot Care Clinic!
