Regular Part-Time (0.8FTE) Registered Nurse – CDM

Are you a Registered Nurse who has a desire to make a difference by working with family physicians to support adults living with chronic conditions? If so, we invite you to apply for this opportunity to join our Chronic Disease Management & Complex Care team in Olds! The position will be a Regular Part-time (0.8 FTE).

Our ideal candidate has:

  • Excellent patient and family engagement skills and knowledge of existing health and community services.
  • Experience working as part of an interdisciplinary team at full scope of practice to provide patient-centered care to specific patient groups with chronic conditions and/or complex care needs (e.g., Diabetes, Hypertension, etc.) is strongly preferred.
  • The ability to work independently, using critical thinking and clinical judgment to coordinate care for patients with chronic care needs in accordance with patient priorities.
  • Experience with adult education and awareness of the principles of self-management and Health Change Methodology.
  • A demonstrated commitment to ongoing professional development and the delivery of evidence-based practice and care planning.
  • Current registration with CARNA, and 3-5 years of nursing experience.
  • Knowledge of and ability to use electronic medical records and be technology savvy.
  • The ability to take initiative, work independently, be a self-starter and self-learner.
