The position will be a Regular Part-time (0.6 FTE) and will be located at the Moose & Squirrel Medical Clinic in Sundre.

As a member of an interprofessional team in the patient medical home, the Nurse Practitioner will work collaboratively with physicians and team members to provide high quality, evidence-based primary health care to an assigned patient population, with a primary focus on patients with complex chronic health concerns.

Our ideal candidate has:

  • Demonstrated experience in successfully working as part of an interdisciplinary team to provide patient and family-centered care in the patient medical home.
  • Previous Nurse Practitioner experience in primary health care setting.
  • Advanced organizational, prioritization and panel managements skills.
  • The ability to take initiative and be a self-starter and self-learner.
  • Decision making, strategic and critical thinking skills.
  • Strong relationship building skills with the team, patients, and their families.
  • Previous experience in using electronic medical records.
  • A Masters of Nursing with a Family Nurse Practitioner focus.
  • Current registration with CARNA.
