Exercise and Active Living

Adults age 18 or over who are interested in improving health through exercise and learning how to increase physical activity safely. People living with chronic conditions such as hypertension, high cholesterol or persistent pain can improve and manage their conditions by increasing their physical activity.

At an individual appointment, our PCN Exercise Specialist will develop an individualized care plan for you based on your goals. You will talk about physical activity and nutrition, receive a “physical activity prescription” tailored for you and your needs, and receive information about local group programs and community workshops that are available to help you sustain active living as a way of life.  Our PCN Exercise Specialist will also talk to you about resources available through Prescription to Get Active.

You can access this service by asking for a referral from your Peaks to Prairies PCN family physician. This service is by appointment only.

Our Exercise Specialist see patients in the clinics located throughout our PCN, or at our PCN location in Olds at 4920-50th Ave. Virtual appointments (phone or Zoom) may also be offered. Days of work may vary depending on location, so please ask your Exercise Specialist about scheduling options when booking your initial appointment.
